WHAT SERVICES DO YOU PROVIDE? My primary services are Biblically based Christian counseling. I provide these services at two locations, Southlake, Texas and Lewisville, Texas. See below for more information about these services. I provide telephone counseling for those who live in outlying or rural areas where Christian counseling is not available. You can find more information about telephone counseling by clicking Phone Counseling. I am available to speak to your men's, women's, or couples' groups. My topics vary but the central topic is based on living a life patterned after the example that Jesus set during his ministry. I will be writing short articles pertaining to healthy mental living from a Christian perspective. Perhaps the very first question most people ask is "What will counseling sessions cost me?" This is a legitimate question. Any organization, whether profit or non-profit, has expenses. When God called us to counseling, He called us to a ministry. And, although the number one concern is the ministry, we still have expenses. We are not supported by any non-profit organizations, groups, or churches and therefore must charge a fee. Southlake Office: The normal fee for each session is $100. We are able to provide a limited number of professional discount fees on an as needed basis. This discounted fee will range from $75-$95. If the normal fee of $100 will create a hardship, it is the responsibility of the client to ask for the professional discount.Lewisville Office: The normal fee for each session is $75. We are able to provide a limited number of professional discount fees on an as needed basis. This discounted fee will range from $50-$70. If the normal fee of $75 will create a hardship, it is the responsibility of the client to ask for the professional discount.Please note that payments are due at time of service. Acceptable forms of payment are cash, check, Master Card, Visa, or Discover. Legal Services: Charges for services such as consultations to attorneys, depositions, to present any or all records pertaining to the counseling relationship to a court official or testify in court proceedings are billed at $200.00 per hour with a minimum of 8 hours, for a total of $1600.00. In the event that any of these situations occur, you, the client agree to pay me for my services, in addition to travel, preparation, and necessary expenditures (copies, parking, meals, etc.) at the rate of $200.00 per hour, rounded to the nearest quarter hour. You, the client, also agree to pay the $1600.00 retainer two weeks prior to the appearance, presentation of records, or testimony requested. I ask that you only request a court appearance in extreme cases. This action will likely result in the need to terminate therapy and I will need to refer you to another therapist. DO YOU ACCEPT INSURANCE? Heart Change Counseling does not accept or file for insurance. However, if your insurance coverage has provision for "Out-of-Network" benefits, we will provide a receipt that is acceptable to your insurance provider if you wish to file a claim. It is the responsibility of the client to know what their insurance benefits are and to file their claims. HOW DO I MAKE AN APPOINTMENT?
You can make an appointment by calling Heart Change Counseling at 469-261-5123. Please leave a message and someone will return your call. If you feel that your call is an emergency, it is advised that you call 911 or proceed to the nearest hospital emergency room. We can also be reached by emailing Heart Change Counseling.
After making your appointment, we recommend that you download the Intake Forms and fill out completely before the Initial Session. This will save time on the first visit. You will need Adobe Reader to download these forms. Adobe Reader is free by clicking the icon below.

DUE AND WHAT FORMS OF PAYMENT DO YOU ACCEPT? Payment to Heart Change Counseling is due at the time counseling is provided. We accept cash, check, Visa, Master Card, or Discover.
WHAT IS YOUR CANCELLATION POLICY?
Appointments cancelled or rescheduled with less than 24 hours notice will be charged the full fee. In cases of emergency or special circumstances where 24 hours notice is not possible, the late cancellation fee may be discussed. The clients will be required to provide a credit card number that can be placed on file and charged in the event of a no-show or late cancellation.
IS MY INFORMATION
CONFIDENTIAL? All information a client shares in therapy, including case notes and records, is confidential. No information will be disclosed without the written permission of the client or a legally authorized person (i.e. parent or guardian). However, this policy does not apply in the following situations: 1. The client states they are going to harm themselves or others. 2. The client expresses knowledge of abuse to a child or senior adult. 3. The counselor is ordered by the court to release records and files.
